Who Is Eligible?

To receive services through Devalor Care Services LLC, individuals must meet the following criteria:

  • Enrolled in NJ DDD's Community Care Program (CCP) or Supports Program (SP) Waiver
  • Diagnosis of intellectual or developmental disability recognized by DDD
  • Active DDD Tier assignment and NJCAT assessment
  • Medicaid eligibility maintained
  • Active Individual Service Plan (ISP) that includes the requested service
  • Support Coordinator assigned by DDD

1. Introduction

Devalor Care Services LLC ("we," "our," or "the Agency") is committed to protecting the privacy and confidentiality of all individuals we serve, their families, and all visitors to our website. 4. Protected Health Information (PHI)

Devalor Care Services LLC handles all Protected Health Information in accordance with HIPAA (45 CFR Parts 160 and 164) and Division Circular #53 of the NJ Division of Developmental Disabilities. PHI will not be disclosed without written authorization from the individual or their legal guardian, except as required or permitted by law, including for treatment, payment, and healthcare operations purposes.

All persons served and their legal guardians will receive a copy of our Notice of Privacy Practices and Rights at the time of admission and annually at their Individual Service Plan (ISP) meeting.

5. Information Sharing and Disclosure

We do not sell, rent, or trade personal information to third parties. We may share information only in the following circumstances:

  • With the NJ Division of Developmental Disabilities (DDD) as required by our provider agreements and applicable regulations
  • With Support Coordinators, healthcare providers, and authorized planning team members for service coordination purposes
  • With law enforcement or government agencies when required by law or to protect health and safety
  • With service partners or contractors who assist in providing authorized services, subject to confidentiality agreements
